Confidentiality is a primary concern of Network Health Systems®
(NHS). This Privacy Statement outlines the information we collect, steps we will take to protect your information and how we will use the information provided to us. When you enter your personal information in our Assessment form, we take your privacy concerns quite seriously, and we take every reasonable effort to protect that information.

II. Information Collected
The only information we collect is the information you provide to us through the use of the
Network Health Systems’ (NHS) Health Risk
Assessment. We do not use “cookies” in any way to identify information about you. Cookies are used inside the assessment specifically tied to the machine, not the user ID and are used to track the user ID’s. This keeps you on track in the assessment itself. Personal information is not stored. Cookie is stored temporarily only – for 20 minutes of inaction or when you log out, if sooner.
In some cases, we may collect information about you that is not personally identifiable. Examples of this type of information include the type of internet browser or computer operating system you are using and the domain name of the website from which you linked to our site. In addition, when you view one of our websites (or sponsors’ information), we may store some information on
your computer. This information will be in the form of a “cookie” or similar file and will help us in many ways. For example, cookies allow us to tailor our website (or sponsors’ information) to better match your interests and preferences. Most internet browsers enable you to erase cookies from your computer hard drive, block all cookies, or receive a warning before a cookie is stored. When you enter your personal information in our Assessment form, we take your privacy concerns quite seriously. We are committed to keeping secure the information you provide to us and take every reasonable precaution to protect your information from loss, misuse or alteration.
Your personal information will not be provided to your employer, your insurer, or anyone else without your expressed authorized permission.
The confidential information that you enter into the Network Health Systems’ (NHS)
Health Risk Assessment remains absolutely private. Your information cannot be accessed by any outside party and is protected by your confidential user name and password.
Confidential information communicated to our servers is encrypted using Secure Socket Layer (SSL) state-of–the-art technology. SSL is the industry standard for secure communication over the Web.
All credit card or banking transactions also use Verisign® and Cybercash® technology to assure confidentiality and security.
All data used is stored behind a secure firewall.
We carefully control access to confidential information. NHS staff members generally do not have any, or need to, access your information as this is all done technologically. NHS staff members who do have access to your information are required to keep this information confidential and are not permitted to use this information for any other purpose than to carry out the services they are performing for NHS. Additionally, we have carefully screened these members and have trained them in the legal nature and medical ethics involved in dealing with this information.
IV. Use of Information
We do not sell or provide names or mailing lists, nor do we provide your name or address to anyone for any marketing or research purposes.
V. Advertisements
Other companies, which place advertising on our site, may collect information about you when you view or click on their advertising or content. We do not control this collection of information. You should contact these advertisers or content providers directly if you have any questions about their use of this information.
